Job Title: Rehabilitative Services Associate (RSA)

Department: Pathways to Success Program
Reports To: Clinic Supervisor
Location: Community-Based / Client Homes / Behavioral Health Center

Position Summary:

The Rehabilitative Services Associate (RSA) plays a key role in the Pathways to Success initiative by providing non-clinical, recovery-oriented support services to youth and families involved in the behavioral health system. Working under the supervision of an LPHA or MHP, the RSA helps clients build life skills, access community resources, and navigate their recovery journey. Services are delivered primarily in home and community settings.


Key Responsibilities:

  • Provide recovery support services such as skill-building, mentoring, goal setting, and support with daily living activities.

  • Help youth and families implement strategies identified in their individualized treatment plans.

  • Support engagement and retention in services by building trusting relationships with clients.

  • Assist families with navigating systems (healthcare, education, juvenile justice, etc.) and accessing community resources.

  • Collaborate with behavioral health teams to coordinate care and share client progress.

  • Model healthy coping skills, boundaries, and relationship-building.

  • Complete service documentation, progress notes, and time logs in compliance with agency and Medicaid requirements.

  • Participate in team meetings, supervision, and required trainings.

  • Uphold trauma-informed, culturally responsive, and person-centered care practices.

Qualifications:

  • Must be a minimum of 21 years of age.

  • High school diploma or GED required; Associate’s or Bachelor’s degree in human services or related field preferred.

  • Experience working with youth and families with behavioral health challenges strongly preferred.

  • Ability to connect with clients through empathy, lived experience, or strong interpersonal skills.

  • Must possess a valid driver’s license, reliable transportation, and willingness to travel to clients’ homes and community locations.

  • Basic computer and documentation skills required.

  • Ability to pass required background checks and complete agency and state training requirements.

  • Knowledge of trauma-informed care and recovery principles.

  • Strong communication, problem-solving, and time-management skills.

  • Experience working in diverse communities.

  • Flexible, dependable, and collaborative team player.

Case Management

About the Role 

Our Case Managers play a key role in helping clients reach their goals by: 

● Assisting with care coordination and linkage to community resources 

● Supporting clients in building life skills, coping strategies, and independence 

● Providing individual and group rehabilitative services (skills training, psychoeducation) 

● Working closely with therapists and medical providers to ensure continuity of care 

● Documenting services accurately and on time 

This is a hands-on position that combines case management, advocacy, and direct client support both in-clinic and in the community.

● Passionate about mental and behavioral health and helping others 

● Organized, dependable, and detail-oriented 

● Skilled at building rapport with clients from diverse backgrounds 

● Able to work independently and as part of a team 

● Comfortable working in the community and supporting clients in real-world settings 

Qualifications 

● Bachelor’s degree in Social Work, Psychology, Human Services, or related field (preferred) 

● Experience working with individuals with mental health or behavioral health needs 

● Valid driver’s license and reliable transportation 

● Ability to pass background check and required trainings
